$90 Medicare payment 2026: didn't get it? cover image

About 20.8 million people with Original Medicare Part B are getting a one-time $90 payment in October 2026, funded by the Medicare Improvement Fund. Most direct deposits from Social Security went out on or around October 8, and paper checks from the Treasury are arriving later in October – so if yours hasn't shown up yet, it may still be in the mail, or you may be in one of the groups that doesn't qualify.

The $90 covers less than half of one month's standard 2026 Part B premium of $202.90. Below is a checklist of every reason you might not have received it, how to check in under ten minutes, and what the payment does – and doesn't – change about your 2027 Medicare costs.

Who gets the $90 Medicare payment

According to the Centers for Medicare & Medicaid Services (CMS) FAQ published October 3, 2026, you are eligible if you are enrolled in Original Medicare Part B, live in the United States, and are not receiving Medicaid help with your Part B premium or paying the Income-Related Monthly Adjustment Amount (IRMAA). The White House fact sheet says the payment draws on the $2 billion Medicare Improvement Fund, created by Congress in 2008 and never used for payments before.

Didn't get it? The 7-point checklist

Check Why it matters What to do
1. Are you in Medicare Advantage? Medicare Advantage enrollees are not eligible Nothing – this payment isn't for you
2. Does Medicaid pay your Part B premium? People with Medicaid premium help are excluded Nothing – your premium is already covered
3. Do you pay IRMAA? Higher-income enrollees paying the surcharge are excluded Check your Social Security benefit letter for IRMAA
4. Do you have Part B at all? It is a Part B premium rebate Check your red, white and blue Medicare card
5. Do you get your benefits by direct deposit? If not, a paper check is mailed – later in October Wait until early November before chasing
6. Has your address changed? Checks go to the address registered with Medicare Update your address with Social Security
7. Do you live outside the US? Recipients must live in the United States Nothing – you're not eligible

Based on the CMS FAQ and White House fact sheet, October 2026.

Still think you're owed it? Call 1-800-MEDICARE (1-800-633-4227) to confirm eligibility, and Social Security at 1-800-772-1213 to check payment status. The paper check carries the memo line "Medicare Improvement Fund Payment; $90 Payment to Offset October Premium" – useful if you're sorting through a pile of mail.

A scam warning. You do not need to apply, pay a fee or give anyone your bank details to receive this money. If someone calls, texts or emails offering to "release" your $90, hang up and call the official numbers above yourself.

What $90 really covers – and what's coming in 2027

The payment is welcome, but it is one-off. Here is how it compares with the Part B numbers retirees will face next year. The 2027 figures are projections from the Medicare trustees, as reported by Checkbook; CMS usually announces the official premium in November.

One-time $90 payment
$90.00
2026 standard Part B premium (per month)
$202.90
2027 projected Part B premium (per month)
$209.50
2027 projected Part B deductible
$292.00

Do the maths and the picture is clear: a projected rise from $202.90 to $209.50 is $6.60 a month, or $79.20 over 2027. In other words, the $90 payment roughly covers one year of next year's projected Part B increase – and nothing more. The Part B deductible is projected to rise from $283 to $292, and the Part D out-of-pocket cap rises from $2,100 to $2,400.

$90 once. $79.20 more in Part B premiums over 2027. The payment pays for next year's increase – it doesn't change your budget beyond that.

This is the part that catches people out. A one-off payment arrives, gets absorbed into the checking account, and three months later the higher premium and deductible are coming out every month with nothing set aside for them.

Three smart things to do with the $90

  • Earmark it for 2027 premiums. Move it to a savings pot labeled "Medicare 2027" – it almost exactly covers the projected annual Part B increase.
  • Use open enrollment. Medicare open enrollment runs October 15 to December 7, 2026. KFF counts fewer Part D plans nationally in 2027 (9 vs 11), and some standalone Part D premiums are jumping sharply, so comparing plans can be worth far more than $90. Our 5 changes to check before December 7 walk you through it.
  • Check whether you qualify for extra help. If money is tight, your State Health Insurance Assistance Program (SHIP) offers free, unbiased Medicare counseling, including whether you qualify for a Medicare Savings Program.
